and Release of Liability form
Return the above documents along with the

• Copy of photo ID or other proof of age, and
• $25 Entry Fee.

Clinic
Classes are free to competitors
Weekly Advanced Class Schedule:
Sunday: Harlem 2:15-4:15
Monday: Chelsea 2pm
Wednesday: LES 1:45pm
Thursday: Midtown 11am
